Personalized PHARE care plan

Description

Objective: to ensure network monitoring of people hereditarily predisposed to cancer (Breast, Ovary, Colorectal, Endometrium). System deployed in 4 territories in France. In recent years, with the Log-After project, this platform has also been developed on a national scale for patients exposed to anti-cancer treatment in childhood.

Context

The device is part of the project LIGHTHOUSE dedicated to managing the patient journey. Designed for a network organization, it allows its stakeholders to collaborate around a shared patient file adapted to the patient's geographic mobility zone.

Expression of need

IS solutions with 4 Phare modules

Evolutionary patient file : specialty files by organ, drawn up with the teams of clinicians, articulated around a common base. Each region can configure new fields, manage its repositories and add new profiles supported by the monitoring module.

Reminder and direct mail: Doctors and the patient are informed and followed up automatically by Phare via mail, secure email or through regional health platforms. The e-mails sent are configurable.

Feedback : depending on their profile and level of access, each stakeholder in the system has global dashboards and a summary of the patient's journey.

Personalized care plan: definition of model protocols associated with given profiles, facilitating appropriate care for each patient.

The protocols are fully configurable by the regions. The inclusion of a patient in a personalized care plan entails the planning of all of his corresponding visits and examinations.

The implementation of the planned plan is monitored, the healthcare network practitioners assigned to the patient have access to the Phare platform to enter the results and submit their reports. Any drift in the initial schedule automatically causes a rescheduling of future examinations or interventions.

Multi-center deployment

For deployment, the Phare Grand Ouest team and its network of eight oncogenetics reference centers use the system at all stages of the care pathway:

  • Patient inclusion
  • Realization of the personalized follow-up plan
  • Follow-up of the examinations carried out and recovery of the results.

Hosting / HDS

Desire for guarantees of hosting that respects the legal framework around the security and confidentiality of health data

  • Genepy network in Midi-Pyrénées: integration into the regional Communicating Cancerology Dossier
  • Close Network (Regional Prevention of Hereditary Cancers in Nord-Pas-de-Calais: 1600 patients already followed, 400 new patients included each year
